ATCNE gladly accepts referrals from other facilities, including courtesy/guest dosing for other outpatient methadone programs.
Helpful documentation to facilitate referrals are:
  • A Release of Information, signed by the client for ATCNE and the referring agency
  • Pertinent medical documentation, including listing of prescribed medications, recent EKG/ECGs, recent History & Physical, diagnoses, and current treatment plans
  • Biopsychosocial documentation, including demographics and substance use history
Referral documentation can be faxed to (617) 782-7668, ATTN: Intake

Referrals are encouraged to contact the clinic directly to complete an initial assessment and to schedule an intake appointment: (617) 254-1271 x119
Please note: photo identification is required for all incoming clients.

Courtesy Dosing

We prefer any requests for courtesy dosing be received one week in advance for in-state and two weeks for out-of-state and that staff confirm directly with ATCNE staff regarding potential courtesy dose requests.

Each client's home clinic must provide the following:
  • A Courtesy Dose Request Form, which must include:
    • Client's full name, date of birth, SSN
    • Current methadone dose level and staff and client contact telephone numbers
    • Potential dates of service for dosing that does not exceed 14 days (extensions after 14 days can be requested, but must include an updated CDR)
    • Signature of the sending program's Medical Director
  • A Release of Information, authorizing the disclosure of information from the home clinic to ATCNE
  • Current list of all medications on file being prescribed to client
  • Methadone dosing history for past ninety days, or complete dose history if admission date is less than ninety days
  • Drug/tox screens for the past six months, or complete history if admission date is less than six months
  • A recent EKG if the client has any of the following:
    • Current methadone dose of 150 mg or higher
    • History of endocarditis, stroke, MI, seizures, and/or irregular heartbeat/palpitations
Please note the following:
  • Courtesy dose requests cannot be initiated on a weekend or holiday
  • Clients must provide a valid, unexpired government-issued photo ID and a last dose letter at their first day
  • There is a dosing fee of $15.00 per day. Payments for weekends/holidays must be arranged in advance.
  • ATCNE does not provide take-home medication to clients from outside facilities, regardless of client's phase-level or dosing schedule

Courtesy dose documentation can be faxed to (617) 782-7668.
